PREVENT / HUMAN APPEAL

A decision about you should be answerable

Use this private, browser-only worksheet to organize facts and draft a respectful request for explanation, human review and correction. It is not legal advice.

First: preserve the decision

Keep the original notice, date, reference number and applicable deadline. Ask for the rules used and available review procedure. Contact qualified legal aid for urgent, high-stakes or time-sensitive matters.

Privacy by design: the worksheet below runs in your browser. It is not sent to FFTAC, and you should not enter Social Security numbers, account passwords, full medical records or other sensitive identifying information.

Build your appeal request

Always check deadlines and the correct appeal address yourself.

What to request

  1. Written notice identifying the decision, its date and who is responsible.
  2. The reasons and relevant records or rules, subject to applicable law.
  3. Whether automated tools contributed to the decision, if the organization can lawfully disclose it.
  4. The existing review, correction, complaint or appeal process and deadlines.
  5. Meaningful human review and repair of any documented error where available.
